Location: Greater Manchester, covering postal areas M8 5RB & M13 9WL
Contract type: Fixed term to 31st March 2025
Hours: 35 per week
Salary: £28,821 per annum based on 35 hours per week
Could you help people who have complex medical / social needs? Would you love to work on an innovative NHS programme that helps support people who access health care more than expected (High Intensity Use (HIU) Programme)?
This is a person-centred service to enable people who need us to achieve their personal outcomes. This could be an enhanced quality of life, increased confidence and independence, and an improved sense of choice and control.
We are looking for 2 Outreach Caseworkers to join our team of experts across Greater Manchester and be a key part of our national network. Help us collaborate on best practices and spread the power of kindness!

What will you bring to the role of Outreach Caseworker?
There is a need to visit some remote locations which have no access to public transport. Therefore you must have a UK driving licence and access to a vehicle which you are willing to use or be eligible for transportation via the Access to Work Scheme.
